1.       One duly completed and signed legalization Application Form

If the applicant entrusts an agent to submit the application on his/her behalf, the applicant and the agent need to sign respectively at the Signature of applicant and at the Signature of agent.

2.     Applicant's original valid passport information page photocopy

3.     The original of the document that has been authenticated by the Ministry of Foreign Affairs DR Congo.

4.     A photocopy of the page with Apostille (signature and stamp) of the document that has been authenticated by the Ministry of Foreign Affairs of DR Congo.

5.     If a document is for commercial affairs legalization, a photocopy of the passport or Election ID card copy of the legal representative of the company or organization, and an official document which proves his or her legal status as the company's legal representative should be provided.

6.     If an applicant entrusts an agent to submit the application on his/her behalf, a photocopy of both the applicant and the agent’s passport or Election ID card should be provided.

7.     The application of Death Certificate legalization should be submitted by a close relative of the deceased. A letter of authorization from the close relative is needed if the application is submitted by a third-party.

8.     The above-mentioned terms are general requirements. Consular officers may require, if necessary, the applicant to provide additional supporting documents or a specific letter of authorization for the agent in case of significant matters.
9.     All rights of interpretation are reserved to the Chinese Embassy in Democratic Republic of Congo .
